An unannounced renewal inspection was conducted today from 10:15am-1:000pm. The physical plant, 3 staff records, 5 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, and eating snack. Hand-washing procedures were also observed. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-60-A Description: Based on record review, 4 of 4 children's records were incomplete. Evidence: 1. Children 1-4's record did not document their start dates (date of enrollment). Plan of Correction: We will start to record this information.
Standard #: 22VAC40-185-80-A Description: Based on record review, the center did not keep a written record of children in attendance each day. Evidence: 1. 1 of 4 classrooms did not have a record of children in care the day of inspection. Plan of Correction: We will remind teachers to take attendance.
Standard #: 22VAC40-185-260-A Description: Based on record review, the center did not have a current annual fire inspection. Evidence: 1. The fire inspection available was dated 10/31/15. Plan of Correction: We will ensure we get the required fire inspection.
Standard #: 22VAC40-185-500-A Description: Based on direct observation, children did not wash their hands before and after eating. Evidence: 1. All children having snack washed their hands before eating but not after. Plan of Correction: We will have children wash their hands after eating.

The injury notification documentation did not include information including: staff present, date and time of parent notification, method of notification, staff and or parent signatures, staff present, or any future action to prevent recurrence of injury. Plan of Correction: We will revise our form.

The tables, used for children to have snack, were not sanitized before and after the children ate. Plan of Correction: We will remind staff to sanitize the tables before and after the children eat.
