22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was initiated today at 1:30pm. The physical plant, 4 staff files, 4 children's files, annual fire inspection, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected today. There were 49 children under the supervision of 8 staff. During the inspection, children were observed being fed (infants), having diapers changed (Toddlers), taking naps, making crafts, listening to music and enjoying free play. For lunch, the children were served chicken, brown rice, broccoli, pears and milk. The interactions between the children and staff were seen to be positive and constructive in nature. Standard #: 22VAC40-185-160-C
Description: Based on file review, 3 out of 4 staff failed to obtain additional TB test/screenings every two years. Evidence 1. Staff #2 went on 8/17 to have a TB test completed. However, there is no documentation that he went back on 8/19 or 8/20 to have the results read to see if the test was positive or negative. 2. Staff #3's most recent TB test/screening was completed on 9/29/14. 3. Staff #4's most recent TB test/screening was completed on 9/22/14

Plan of Correction: Staff will complete new TB test/screenings and bring in documentation.

Standard #: 22VAC40-185-70-A
Description: Based on file review, 1 out of 4 staff records does not have all required information on file. Evidence 1. Staff #4 does not have documentation to demonstrate that the individual possesses the education, orientation training required by the job position.

Plan of Correction: The director will ask the staff to bring in education verification. The New employee checklist will be completed.

Standard #: 22VAC40-185-270-A
Description: Based on observations, there were areas of the center which were not maintained in a safe, operable condition. Evidence 1. In the school age classroom:, the - there was a seat cushion that had been ripped open at the seam. exposing the inside material. - the wall of the cabinet, which encases a sink (closest to the girl's bathroom), was deteriorating

Plan of Correction: We will replace the cushion. A work order will be placed for the maintenance to fix/replace the cabinet.

Standard #: 22VAC40-185-450-A
Description: Based on interview and observations, there were children sleeping during the designated rest period without linens accessible for use. Evidence 1. In the school age classroom. there were two students asleep on cushioned couches. The cushions did not have linens, but the children had their coats draped over them. When asked if there were any linens available to the children, the teacher stated that they did not have linens, which is why she had them use their coats.

Plan of Correction: The director will ask the parents to provide linens.

Standard #: 22VAC40-191-40-D-3-D
Description: Based on file review 1 out of 5 staff files does not have background checks completed before three years since the last checks were completed. Evidence 1. Staff #1's most recent completed criminal history record check (CRC) was last completed on 10/11/12 and her most recent search of the central registry (SCR) was last completed on 11.26.13. 2. Staff #2's most recent SCR was last completed on 9/16/13.

Plan of Correction: These will be completed again.
