Comments:
An unannounced monitoring inspection was conducted today 2/7/2017. There were 36 children in care with four staff members. The inspector arrived at 8:30 a.m. and departed the program at 12:00 p.m. The inspector reviewed four children's records during the inspection and four staff person's record. The inspector observed the children during: circle time, breakfast, free play in centers, assessments, lunch and art time. Discussion with the staff about : areas and equipment maintained in a operable, safe condition.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on a review of four staff's records, the center failed to ensure that the Center retain information at the center of the name, address and telephone number of a person to be notified in an emergency for each staff . Evidence: 1. Documentation of all four staff members were not available for review.

Plan of Correction: The administration staff will make sure that each staff's person emergency contact and required information is at the center.

Standard #: 22VAC40-185-270-A
Description: Based on observation of the areas and equipment, the center failed to ensure that all areas and equipment inside and outside was maintained in a clean, safe and operable condition. Evidence: 1. The inspector observed three protruding nails that were exposed on the deck where the children enter the program. They pose a hazard to the children.

Plan of Correction: The administration will have the nails removed or hammered down. The teachers will monitor all areas.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the center failed to ensure that the most recent violation notice was not posted. Evidence: 1. The most recent violation notice was not posted from 9/2016. The posted violation notice was dated 4/26/2016.

Plan of Correction: The teachers will post the violation notice from today's inspection. The administration will make sure that each site receives a copy.
