22VAC40-191 Background Checks (22VAC40-191)
The licensing inspector conducted an unannounced monitoring inspection from 9:20am - 12:30pm. There were thirty preschool children in care: sixteen children with two staff in classroom #1 and fourteen children with two staff and one volunteer in classroom #2. One additional staff person was also present. The children were observed playing in the centers, at circle time, dancing to music and preparing for lunch. Two staff and five children's records were reviewed. Areas of non-compliance are identified on the violation notice. Standard #: 22VAC40-185-60-A Description: Based on record reviews, the center failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed five children's records. One of five records reviewed (child record #5) had the custodial parent requesting the center not to release the child to the other parent; however, there was no legal paperwork in the child's record. Plan of Correction: The teacher will contact the custodial parent and inquire about the legal paperwork.
Standard #: 22VAC40-185-510-E Description: Based on review of the medication, the center failed to obtain medication labeled with the required information. Evidence: 1. The inspector observed the medication. There was a Proair inhaler for child #2; however, the medication was not labeled with the child's name, dosage amount or the times to be given. The medication has not been administered. Plan of Correction: The teacher will contact the child's parents and request the original container which will include this information.
Standard #: 22VAC40-185-550-D Description: Based on review of documentation, the center failed to document any shelter-in-place drills for the calendar year 2016. Evidence: 1. The inspector reviewed the documentation for emergency and shelter-in-place drills. There were no shelter-in- place practice drills documented for 2016 for classroom #1. Plan of Correction: The teacher said at least one shelter-in-place drill was practiced in 2016, but not documented. The teacher will ensure at least two shelter-in-place drills will be practiced and documented for this year.
