Description: The center failed to get a subsequent Child Protective Service check done for one staff. Evidence: Based on the review of 3 staff records, one staff, staff C is missing the subsequent CPS check. The last check was conducted on 2/4/2014.

Plan of Correction: We will make sure we send in the paperwork.

Description: The school age room has cleaners and insecticide spray that are accessible to the children . Evidence: Based on the physical plant review , the school age room has insecticide and cleaners stored on a shelf next to the sink area. The cleaners and insecticide are not stored in a locked area.

Plan of Correction: We will remove and place in a locked cabinet.

Standard #: 22VAC40-185-550-C
Description: The center failed to add the shelter in place space to the emergency map. The map is also not posted in the school age room. Evidence: Based on the physical plant review of the center , the map for the emergency evacuation does not include shelter in place and the school age room downstairs does not have any maps posted.

Plan of Correction: We will add the shelter in place to the map and post one in the school age room.
