Comments:
An unannounced renewal study was conducted from 10:15 a.m.-12:30 p.m. with the owner/primary directress. 23 children, ages 17 months through 6 years old, were under the care and supervision of five teachers. Children were observed playing outside, listening to a readaloud, and having work time in different areas of the Montessori environment. Both the fire and health inspections are current. The last emergency escape drill was 1/18/17. The last shelter-in-place drill was 10/27/16. The complete first aid kit, flashlights, and battery operated radios were present. There were no medications to review. Four children's records and four staff records were reviewed. Standard #: 22VAC40-185-60-A
Description: Based on review of four children's records, the center did not maintain complete children's records. Evidence: Child A's record did not include documentation of viewing proof of the child's identity and age.

Plan of Correction: Child A's parents did provide this information, and we will locate this information and insert it into the file.

Standard #: 22VAC40-191-60-B
Description: Based on review of four staff records, the center did not have original sworn disclosure statements for corporation officers. Evidence: Corporation Officer A's and Corporation Officer B's records did not include sworn disclosure statements.

Plan of Correction: The owner/primary directress stated that the background checks were completed, and once they are located, she will present them to the licensing inspector.

Standard #: 22VAC40-191-60-C-1
Description: Based on review of four staff records, the center did not have original criminal history record check reports for corporation officers. Evidence: Corporation Officer A's and Corporation Officer B's records did not include returned, criminal history record check reports.

Plan of Correction: The owner/primary directress stated that the background checks were completed, and once they are located, she will present them to the licensing inspector.

Description: Based on review of four staff records, the center did not have original central registry record checks for corporation officers. Evidence: Corporation Officer A's and Corporation Officer B's records did not include returned, central registry record check reports.

Plan of Correction: The owner/primary directress stated that the background checks were completed, and once they are located, she will present them to the licensing inspector.
