COMPLAINT # 47104 An unannounced complaint investigation was initiated today to review issues related to the complaint. Staff were interviewed and classrooms areas, changing tables, eating areas, eating utensils and bathrooms were inspected. The following standard areas were inspected: Special Care Provisions: 490: Preventing the spread of disease; 500: Handwashing and toileting procedures; 560: Nutrition and food service The following standard was found to be in violation: 490: The cleaning and sanitation of a surface contaminated with body fluids. Therefore, the complaint is determined to be valid. A violation notice was issued, and the director has sent in the responses. If you have any questions, please contact me at

Complaint related: Yes
Description: During today's inspection, a surface contaminated with body fluids was not sanitized as required to prevent the spread of disease. Evidence: During today's visit, the one toilet seat used by the two years old classroom was observed to have 12 square adhesive stickers on the seat, causing it to be porous and therefor unable to be sanitized. Staff reported that they placed stickers on the toilet to encourage toilet training.

Plan of Correction: The stickers were removed from the toilet seat and it was sanitized.
