An unannounced renewal inspection was initiated today at 1:30pm. The physical plant, 4 staff files, 4 children's files, annual fire inspection, staff training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected today. There were 37 children under the supervision of 5 staff. During the inspection, children were observed taking naps during rest time, cleaning up and eating afternoon snack, enjoying free play. The children bring their lunches from home each day they are in care. The afternoon snack for today was pretzels and milk. The interactions between the children and staff were seen to be positive and constructive in nature. There are currently 3 staff certified in MAT (medication administration training). Description: Based on file review, 1 out of 4 children's records does not have a physical examination. Evidence 1. Child #1 does not have a physical on file.

Plan of Correction: We will contact the parents to get page 4 of the Health form.

Standard #: 22VAC40-185-70-A
Description: Based on file review, 1 out of 4 staff records does not have all required information. Evidence 1. Staff #1 does not have written information to demonstrate that the individual possesses the education required by the job position.

Plan of Correction: We will get documentation (diploma or transcript) of her education.

Standard #: 22VAC40-185-210-A
Description: Based on file review, 1 out of 4 staff records does not have documentation of being qualified to be a program leader. Evidence 1. Staff #1 does not have documentation of education and experience to support being qualified to be a program leader.

Plan of Correction: The education transcript (once obtained) will meet these qualifications.

Standard #: 22VAC40-185-550-M
Description: Based on file review, the written record of injuries (serious and minor) a child may sustain while in care at the center does not have all required information. Evidence 1. The Accident Report does not include documentation of any future action to prevent recurrence of the injury.

Plan of Correction: This will be added to the Accident Report.
