Reviewed standard 22VAC 40 -185-340 F and G.
An unannounced monitoring inspection was conducted on February 8, 2017 from approximately 3:15 pm to 5:45 pm. Three staff were present with thirty - six children. Children were observed during snack, bathroom break, and multiple stations. Three children records, two medications records, injury records were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, hand washing procedures, nutrition, injury report documentation, children's records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, emergency supplies and required postings. Exit interview conducted with program director.
Standard #: 22VAC40-185-280-B Description: Based on observation, the licensee did not ensure all hazardous substances were kept in a locked place. Evidence: During the inspection conducted on February 8, 2017, the Licensing Inspector observed in a unlocked closet two bottles of hand sanitizer labeled with "keep out of reach of children" and "warning". The closet is not used by the program but children have access to the unlocked closet in the cafeteria. Plan of Correction: Not available online. Contact Inspector for more information.

Evidence: On February 8, 2017, Licensing Inspector reviewed injury report forms child #1 injury report did not did not have documentation of future action to prevent reoccurrence of injury. Standard #: 22VAC40-185-560-F Description: Based on observation, the licensee did not ensure food containers shall be sealed and clearly dated and labeled in a way that identifies the owner. Evidence: During the inspection conducted on February 8, 2017, the children were observed eating snack. Three children were eating there snack from home. The licensing inspector observed and interviewed the children, 3 of 3 lunch bags were not labeled with a date and 2 of 3 lunch boxes did not have a name. Plan of Correction: Not available online.
