Review standard 22 VAC 40-185-60 A 13 and 14.

An unannounced inspection was conducted at the facility on February 23, 2017, from approximately 2:15 pm - 3:45 pm. Upon entrance there were 24 children and 5 staff . The Licensing Inspector observed toileting, manipulative, nap table top toys and free playing. The Licensing Inspector reviewed 3 children records and 1 staff records. Medication is not given at the center. Licensing Inspectors reviewed supervision, activities, equipment, nutrition, injury report documentation, children and staff records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, liability insurance coverage, emergency supplies and required postings. The exit interview was conducted with director.

Description: Based on 1 out of 3 records reviewed, the licensee did not ensure the record contained all required documents. Evidence : During the inspection conducted on February 23, 2017, child #3 did not have one of the emergency contact addresses.

Plan of Correction: The owner will contact parent to get address.

Standard #: 22VAC40-185-440-F
Description: Based on observation, the licensee did not ensure the rest mats can be sanitized on all sides weekly. Evidence: During the inspection conducted on February 23, 2017, 6 of the rest mats had holes that exposed the foam padding.

Plan of Correction: The owner will replace the rest mats.

Description: Based on observation the license did not ensure the diaper ointment was in the original container and labeled with the child's name. Evidence: During the inspection conducted on February 23, 2017 , there was 1 of 2 diaper cremes were not labeled with child's name.

Plan of Correction: Corrected when inspectors was present.

Description: Based on record review ad interview, facility did not ensure 1 of 5 injury records have documentation of required information. Evidence : On February 23, 2017 , licensing inspector reviewed the 3 year old classroom written record of injury for February 14, 2017, November 10, 2016 and November 8, 2016. The written injury record did not contain time parents were notified.

Plan of Correction: The owner addressed the staff about documenting time parent was notified.
