Technical Assistance:
Discussion on posting location of license and most recent licensing inspection. Discussed importance of labeling/dating bottles and food brought from home.

The following Monitoring Inspection took place between the hours of 12pm to 2:55pm. Today, I met with the Staff of the Chesterbrook #826 located in Ashburn, Virginia. At the start of this unannounced monitoring inspection there were a total of 123 children with 18 staff; good ratios in all 9 rooms. School age classroom was not occupied during the inspection. A total of 9 children?s files and 6 staff files were reviewed. The children were engaged in the following: transitioning from lunch, handwashing was observed after lunch and preparing for nap. Standard #: 22VAC40-185-240-D-4
Description: Based on staffing at the beginning of the inspection at staff member was not present who has current MAT training. Evidence: The center currently has children enrolled with emergency medications, when the inspector arrived, no staff present with current MAT training present.

Plan of Correction: One staff on site who's MAT training expired and the Principal was at a meeting. The staff with the expired training has been enrolled in the next MAT training. Center will ensure that someone with current training will be on site at all times.

Standard #: 22VAC40-185-270-A
Description: Based on observation of physical plant there were areas of the building that were not maintained in a clean, safe and operable condition. Evidence: The blinds in the Beginner's classroom was torn across the bottom. A red chair and blue couch in both the Beginner's and the Pre K classroom were found torn in several areas.

Plan of Correction: Principal stated that the blind has been glued several times and will re-glue the blind to repair. The furniture will be removed and replaced.

Description: Based observation of physical plant, hazardous substances were accessible to children. Evidence: A spray bottle containing bleach was found in an unlocked cabinet off the K/school age classroom which was accessible to children.

Plan of Correction: Staff removed the cleaning bottle immediately.

Standard #: 22VAC40-185-500-B
Description: Based on observation of diapering area two areas for diapering no longer have a nonabsorbent surface for diapering. Evidence: The diaper changing mats in the Toddler B and Beginners classroom were torn and no longer nonabsorbent.

Plan of Correction: Principal will have both mats replaced today.

Standard #: 22VAC40-185-510-N
Description: Based on review of the center's medications, medications for four children were not returned or disposed of. Evidence: Child #3's Motrin authorization expired on 11/7/2016, the medication remains on site. Child #4's inhaler expired 10/2016, this medication remains on site. Child #4's EpiPen expired on 8/2016 and the physician authorization expired on 1/6/2017 and Benadryl for Child #4 expired on 12/16, both medications remain on site.

Plan of Correction: Principal has removed all medications mentioned and will return all medications to parents today.

Standard #: 22VAC40-185-560-G
Description: Based on observation food brought from home was not labeled. Evidence: Juice boxes brought from home for child # 1 and Child #2 were not labeled with name and date.

Plan of Correction: Staff will ensure that parents will label date any food brought from home.

Standard #: 22VAC40-185-570-E
Description: Based on observation of the refrigerator in Infant A classroom, infant formula was not dated. Evidence: Multiple bottles for three different infants were not dated.

Plan of Correction: Staff will ensure that parents date the bottles that are brought in daily.
