Comments:
An unannounced monitoring inspection was conducted with the director beginning at 12:00 PM and ending at 2:20 PM. There were fourteen children, ages three and four years old, in care during the inspection. The children were observed taking individual and class portraits for picture day. The children were also observed playing Simon Says. Eight children's records and three staff records were reviewed. The emergency escape drill logs were reviewed and determined to be in compliance. The most recent fire inspection was documented as August 4, 2016. The most recent health inspection was documented as August 8, 2016. The first aid kit, flashlight, weather-band radio were observed and in compliance. Standard #: 22VAC40-185-60-A
Description: Based on a review of eight children's records, not all the records were complete. Evidence: Child A's record did not contain documentation of proof of the child's identity and age. Child B's record was missing the addresses for two emergency contacts.

Plan of Correction: The center will obtain the missing information from the parents.

Standard #: 22VAC40-185-240-D-5
Description: Based on observation of six staff records it was determined that that there is not at least one staff member on duty who is trained within the last three years in performing daily health observation of children. Evidence: The most recent date of daily health observation training reviewed was 06/10/2013.

Plan of Correction: The center will schedule for staff to be trained in daily health observations.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on a review of three staff records, the center did not obtain updated background checks every three years as required. Evidence: Staff A's most recent criminal background check was dated April 3, 2012.

Plan of Correction: The center will complete an updated criminal background check.
