An unannounced investigation was conducted to investigate a compliant regarding staff-to-children ratios and failure to notify parent of a child?s injury. The investigation was conducted with the assistant director on February 21, 2017 beginning at 6:45 AM and ending at 8:00 AM. A Staff interview was conducted February 21, 2017 beginning at 3:34 PM and ending at 4:20 PM. Classroom observations were conducted and staff-to-children ratios were in compliance during the investigation. Two staff interviews were conducted. Two staff records and two children's record was reviewed and determined to be in compliance. The incident and accident records were reviewed. Based on the information obtained during the investigation the complaint was determined to be valid. Standard #: 22VAC40-185-550-L Complaint related: No Description: Based on staff interviews and review of documentation, the facility failed to notify the parent by the end of the day of any known minor injuries. Evidence: Child A sustained a minor injury on February 1, 2017 and the parents were not notified of the injury by the end of the day. Plan of Correction: The assistant director met with facility staff to review the policy of documenting and notifying parents of accidents and injuries at the facility prior to the investigation.

Evidence: Child A sustained a minor injury on February 1, 2017 while at the facility and there was no written record of the incident. Plan of Correction: The assistant director met with the facility to review the policy of recording serious and minor injuries when they occur prior to the investigation.
