Standard #: 22VAC40-185-60-A Description: Based on record review, 3 of 3 children's records were incomplete. Evidence: Proof of Identity documentation did not contain the date and name of the staff member who viewed the required document. Based on medication review, 1 child's allergy action plan was not accurate. Evidence: The plan calls for the center to administer an antihistamine if an allergy reaction is happening but the antihistamine is not on-site for child #1. Plan of Correction: The center director will sign and date birth certificates or other form of identity when it is viewed during registration. The center director will contact the parent to bring in the antihistamine or update the action plan as needed.
Standard #: 22VAC40-185-280-B Description: Based on observation, two of seven classroom cabinets were unlocked that contained hazardous substances. Evidence: Cabinets were unlocked in two classrooms which contained bottles of cleaning solutions for the tables. The locks were on the cabinets but were not in use to prevent children from opening the doors. Plan of Correction: All staff will be reminded to lock cabinets at all times.
