Comments:
An unannounced monitoring inspection was conducted in the Center on January 26, 2017. There were 16 children and 3 staff members present. Reportedly, 21 children are enrolled in the program. Upon arrival to the Center at approximately 11:15 AM, the children were observed playing outdoors. The children were also observed during the lunch meal, nap/rest time, and afternoon snack. In the afternoon, the children returned outdoors for play. The dismissal procedures were also observed. A discussion was held regarding the emergency drills, the times shall vary to include the school-age children in the practiced drills. A sample of four records was reviewed, 3 children's and one staff. Emergency medications were observed in the Center. Findings of the inspection were reviewed with a staff member. Description: Based on reviewed documentation, the Center failed to obtain documentation of additional immunizations for children once every six months under the age of two years. Evidence: Documentation of the immunization record for child #1 was not up to date. The date of the last immunization was noted as 11/15 when the child was 15 months old.

Description: Based on a sample review of 3 children's records, the Center failed to obtain documentation of a physical examination for an enrolled child: before the child's attendance; or within one month after attendance. Evidence: Documentation of a physical examination was not available for review for child 1, the child has been enrolled over 30 days, date of attendance 08/2016.

Standard #: 22VAC40-185-270-A
Description: Based on direct observations by the LI, the Center failed to ensure areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: The LI observed children as they played on outdoor equipment located on the playground. Sections of the equipment was tied together with rope. The composite equipment was not level on the ground; three children were observed standing on the equipment and caused it to rock from side to side which presented a risk of turning over.

Description: Based on observations and staff report, the Center failed to ensure staff have completed medication training to administer prescription medications that would normally be administered by a parent or guardian to a child. Evidence: 1. Based on interview, Staff reported that staff have not completed medication administration training nor meet the requirements in 22VAC40-185-240D. 2. The LI observed prescribed emergency medications in the Center for 3 children. Two of the 3 medications had expired.

Standard #: 22VAC40-191-40-D-1-B
Description: Based on review of a staff record, the Center failed to ensure a background check was verified before 30 days of employment at the facility ends. Evidence: Documentation of a verified Search of the central registry was not available for review in the record of staff #1. The employee was employed by the Center in September, 2015. A document which was returned from the responsible agency was observed in the record. The document noted not processed and was dated 10/8/2015. It does not appear the document was resubmitted for verification.
