An unannounced monitoring inspection was conducted today. There were 3 children and 2 staff present at LI arrival. 2 staff and 2 children's files were reviewed. See violation notice for missing information. Children were observed playing, washing hands, during a diaper change, eating lunch and resting quietly. Emergency escape drills, shelter-in-place drills, first aid supplies, labeled lunches were all in compliance. If you have any questions regarding this inspection, please contact me at 703-479-4678. Standard #: 22VAC40-185-60-A
Description: Based upon a review of files, one child's file was incomplete. Evidence: 1 of 2 files reviewed was missing documentation of the mother's work information. ( Child #1)

Plan of Correction: The parent has been asked to provide this information.

Standard #: 22VAC40-185-240-D-5
Description: Based a review of staff files, no staff members on duty with children had obtained Daily Health Observation training. Evidence: At LI arrival there were 2 staff present. No staff person has obtained Daily Health Observation training.

Plan of Correction: Staff are signed up for the course.

Standard #: 22VAC40-191-40-D-1-A
Description: Based upon a review of files, one staff file was incomplete. Evidence: Staff A did not have a completed Central Registry Search on file within 30 days of employment. (Date of Hire :12/07/16) The search results on file are dated 1/27/17.

Plan of Correction: I called Richmond to inquire about the search; however, I failed to document this in the file. I will maintain documentation in the file in the future.

Description: Staff A was allowed to work, past 30 days of employment without a completed CPS search on file.

Plan of Correction: Staff will not be allowed to work without a completed search on file.
