An unannounced complaint inspection was conducted on 2/15/2017 regarding supervision and injury documentation. Based on the information gathered the allegation regarding inadequate supervision is found to be not valid. The allegation regarding injury documentation is found to be valid. The violation of Standard 550.M is listed on the violation notice and was discussed with the center director at the exit interview.
Standard #: 22VAC40-185-550-M Complaint related: Yes Description: Based on record review and interview, the center failed to maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include staff and parent signatures or two staff signatures. Evidence: 1. Child #2 received an injury on 1/30/2017 at approximately 9:30am. This injury was confirmed by staff #1. a. Staff #1 and staff #2 stated they could not locate the written injury report for this injury. 2. The written injury report for child #1 lacked a parent signature or second staff signature. a. The injury occurred on 1/30/2017 at 9:15am and the injury report was signed by staff #1. b. Staff #1 reported the report was given to the parent who refused to sign the report. Plan of Correction: Moving forward management and staff to ensure incident/accident reports to be completed anytime a child sustains an injury.
