Licensing Inspector arrived at 11:15 AM and departed at 1:30 PM. At the time of the tour, there were 51 children with 10 staff. Children were observed eating lunch, reading stories, and napping. Infant care, medication and center documents were also reviewed. The sample size was four staff and six children's records. Violations were noted in the areas of administration and special care provisions & emergencies.
Standard #: 22VAC40-185-130-B Description: Based on review of six children's files, it was determined that the facility did not ensure that immunizations were obtained once every six months for children under two years of age. Evidence: The most recent immunizations available for child #2 (date of birth 2/17/16) were dated 8/29/16. Additional immunizations should have been obtained in February of 2017. Plan of Correction: Assistant director is in the process of auditing the children's records and will reach out to the parents to obtain needed information.
Standard #: 22VAC40-185-60-A Description: Based on review of six staff children's records, it was determined that the facility did not ensure that all elements were present in each child's record. Evidence: 1. The record for child #2, who was in care at the time of the inspection, did not have addresses for either of the emergency contact persons. 2. The record for child #3, who was in care at the time of the inspection, did not contain the city and states for the emergency contact persons. Plan of Correction: Assistant director is in the process of auditing the children's records and will reach out to the parents to obtain needed information.
Standard #: 22VAC40-185-550-L Description: Based on review of injury reports, it was determined that the facility did not ensure that parents were notified of any known minor injuries by the end of the day. Evidence: The incident report for child #7 indicated that he was injured on on 2/28/17 and that the parents were not notified of the injury until 3/1/17. Plan of Correction: Center director will review injury procedures with staff to ensure that all injuries are reported on the same day.
