We discussed the requirements for updating sworn statements or affirmations. The risk assessment for a violation was provided.
We talked about injury documentation, annual (or more) reviews of the injury prevention procedures, including the review of injury records, program and services. Also, we talked about procedures if a change with the terms of the license and operation will occur.
Please, give attention to email notification for changes with criminal record checks for staff.
Thank you for your assistance during today's unannounced monitoring inspection conducted from 10:40 AM to 2:35 PM. Today, there were fourteen to fifteen children, ages three to five, with two to three staff. I viewed program activities, staffing, supervision, interactions with children, equipment, posted information, agency inspections, injury records, emergency procedures and drill records, five staff records, staff training, three records for children, medication record for one child, daily routines, lunch, indoor and outdoor areas. Today, the children enjoyed group activities such as language/spanish, music, and an interactive alphabet smart board activity. There have been regular opportunities for age appropriate choice of classroom interest areas, art, stories/books and active play outside or in the gym. Standard #: 22VAC40-191-40-D-1-C Description: Based on a review of five records for staff, and an interview with administrative staff, there was not an updated sworn statement or affirmation in the file for staff member 3. Evidence: The most recent sworn statement in the file was completed on June 3, 2013. The updated criminal history record and central registry search were completed before the three year due dates in 2016 for staff member 3 and the statement could not be located. Plan of Correction: An administrator immediately had staff member 3 complete an updated sworn statement or affirmation form. In the future, administrative staff will make sure the sworn statements are completed at the same time the request forms are prepared for updating background checks.
