An unannounced renewal inspection was completed on March 7, 2017. The purpose of this visit was to measure the facility?s compliance with the licensing standards. The following ratios were observed: In Toddlers/Young preschool-3:12; Preschool-1:9. The toddlers were being diapered. The preschool children were engaged in free choice activities during center time. The preschool children were also observed playing outside. The children ate chicken tenders, green beans, potatoes and milk for lunch. The facility administers only emergency medication. The first aid kit and emergency supplies were inspected and found to be complete. The last evacuation drill was conducted 2/28/17. The most recent shelter in place drill was conducted 7/19/16. Three children?s records and three staff records were reviewed. Approximate time on site: 10:30am to 1:20pm
Standard #: 22VAC40-185-160-A Description: Based on a review of a staff person's record, the facility did not ensure that a staff person's tuberculosis (TB) screening was conducted within the required time frames. EVIDENCE: The x-ray for staff person #2, start date 10-26-16, was completed on 8-19-15, more than 12 months prior to her date of employment. TB screening must have been completed within 12 months prior to or 21 days after the date of employment. Plan of Correction: I will make sure the staff person gets a current TB screening.
Standard #: 22VAC40-185-70-A Description: Based on a review of a staff person's record, the facility did not ensure that verification of education was in her record. EVIDENCE: The record of staff person #2, start date 10-26-16, did not contain verification of education to qualify her for her position. Plan of Correction: Will verify the staff person's education and place in her record.
Standard #: 22VAC40-185-270-A Description: Based on observations at the facility, the facility did not ensure that all areas and equipment, inside and outside of the facility,were maintained in a safe condition. EVIDENCE: 1)The inspector observed a bookshelf, that is over 5 feet tall, in the toddler room that has become unattached from the wall. This presents a tip hazard should a child pull or climb on the shelves. 2) The inspector observed a broken board on the parking lot side of the playground fence. The broken wood has jagged edges that present an injury hazard. Plan of Correction: We will replace the board in the fence. The bookshelf will be reattached to the wall.
Standard #: 22VAC40-185-330-B Description: Based on an inspection of the playground area, the facility did not ensure that resilient surfacing was placed in a fall zone area surrounding equipment with moving parts. EVIDENCE: The inspector observed children playing on two pieces of equipment with moving parts: a bicycle-go-round and the other a playground roundabout (merry-go-round). There was no resilient surfacing under or around this equipment. Resilient surfacing must be under and surrounding equipment with moving parts maintained in a fall zone area sufficient enough to include the child's trajectory in the event of a fall. Plan of Correction: Equipment will not used until the resilient surfacing is in place.
Standard #: 22VAC40-185-500-A Description: Based on observations in the toddler room, the facility did not ensure that children's hands were washed after being diapered. EVIDENCE: The inspector observed two toddlers being diapered. After their diapers were changed, the staff person did not wash the children's hands. Plan of Correction: Staff will be retrained on hand washing today.
Standard #: 22VAC40-185-500-B Description: Based on observations in the toddler room, the facility did not ensure that the diaper changing surface was cleaned properly between use. EVIDENCE: The inspector observed the staff person spray the changing table with Lysol disinfectant spray after changing a child and immediately wiping with a paper towel. The diapering surface must be cleaned with soap and at least room temperature water and sanitized after each use. Plan of Correction: Staff will be retrained on proper diapering procedures today.
