An unannounced monitoring inspection was conducted on March 8, 2017 from approximately 10:20 am to 1:20 pm. Six staff were present with thirty-two children. Children were observed participating in various teacher guided activities as well as free play. Licensing Inspector reviewed standards related to supervision, activities, equipment, supplies, nutrition, special feedings, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, injury reports, liability insurance coverage documentation, children & staff records, emergency supplies and required postings. Violations were reviewed with and verified by the program director during the exit interview conducted on this date.

Standard #: 22VAC40-185-340-F
Description: Based on observation, facility did not ensure children under 10 years of age are alvways within actual sight supervision of staff except while a child is using the restroom. Evidence: On March 8, 2017 at 12:13 pm, staff #2 was observed cleaning the classroom bathroom while 8 children were napping in the 2's classroom. No other staff was present in the room therefore children were not in sight supervision of staff.

Plan of Correction: Program Director will speak with staff in regards to sight and sound supervision of children at all times.

Standard #: 22VAC40-185-440-E
Description: Based on observation, facility did not ensure there is at least 12" of space between occupied cots. Evidence: On March 8, 2017, the following was observed: 1. In the pre-k classroom, 4 children's cots were approximately 4" - 6" apart from each other. 2. In the 3's classroom, 4 children's cots were approximately 4" - 6" apart from each other.

Plan of Correction: Program Director will ensure cots are the appropriate 12" during nap time.

Description: Based on review, facility did not ensure 2 shelter-in-place drills are conducted each year. Evidence: On March 8, 2017, there was not documentation of 2 shelter-in-place drills in 2016.

Plan of Correction: Program Director will ensure shelter-in-place drills are consistently monthly.

Standard #: 22VAC40-185-550-M
Description: Based on reviewed, facility did not ensure 2 of 6 injury reports have documentation of required information. Evidence: On March 8, 2017, the following was found: 1. A report completed on January 30, 2017 did not document the time parent was notified. 2. A report completed on February 21, 2017 did not document the date parent was notified.

Plan of Correction: Program Director will ensure staff are filling out the injury report completely to include parent's notification time and date.
