22VAC40-191 Background Checks (22VAC40-191)
An unannounced renewal inspection was conducted today from 10:10am-11:45pm. There were 13 children directly supervised by 4 staff. The physical plant, 2 staff records, 3 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in outdoor group play, bottle feeding. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-270-A Description: Based on observation, areas of the center were not all maintained in a clean, safe or operable condition. Evidence: 1. A magnetic drawing toy in the preschool room was observed to have a puncture. 2. The outdoor swings have plastic coatings on the chains that are peeling and chipping and shredding creating sharp edges. 3. The right structure beam on the ladder leading to the bridge section of the outdoor play structure has a large chunk of rotted wood missing and exposing splintered edges. 4. A play may was observed in the toddler room with rips and tears on the edges. The mat was also stretched and warping creating a tripping hazard for children and staff. 5. The base boards in the school age room were observed to have chipping and peeling paint. 6. Areas of the plastic coated chain link fencing around the playground have become stripped and warped exposing several links of sharp metal fencing. Plan of Correction: The mat and the broken toy were removed during the inspection. The issues with the playground will be discussed with the supervisor and fixed as soon as possible.
Standard #: 22VAC40-185-290-3 Description: Based on observation, not all electrical outlets had protective coverings installed. Evidence: An outlet in the toddler room to the right of the changing table was observed without an protective cover. Plan of Correction: Corrected during inspection.
