Description: Based on a review of records on 03/06/2017, the center did not ensure that a new applicant had a sworn statement or affirmation when the applicant changed. EVIDENCE: The record of Board Member #2 did not have a sworn statement or affirmation.

Plan of Correction: Will have board member sign tomorrow

Description: Based on a review of records on 03/06/2017, the center did not ensure that any applicant has a sworn statement or affirmation, before three years since the dates of the last sworn statement or affirmation. EVIDENCE: The most recent sworn statement for Board Member #1 is dated 09/14/2012.
