Comments:
An unannounced non-mandated monitoring inspection was completed on 3/10/17 as a part of the licensee's probationary status. There were 17 children present in two groupings with three staff directly supervising. Three children's records and two staff records were reviewed. No medications at the site. The inspector arrived at the center at 11:15 am and departed at 4:30 pm. The children were observed finishing lunch and during nap.

Standard #: 22VAC40-185-140-A
Description: Based on record review, the center failed to ensure that one of two newly enrolled (over 30 days) children had a physical examination by or under the direction of a physician before attendance or within 30 days after attendance. Evidence: 1.There was no physical examination in the record for child #2 (20 months old). Child #2 first date of attendance was 2/2/17 and was present during the inspection.

Plan of Correction: The front desk operator contacted the parent during the inspection to request a physical examination be provided by Monday. The front desk operator will ensure that a complete health record is provided to the parents and completed prior the child's first date of enrollment. The director will change the child file checklist to include all parts of a health record. The director will check all new children's files to ensure they are complete.
