An unannounced annual subsidy health and safety inspection was completed on 3/10/17. There were 17 children present from 20 months through five years. Three children's records and two staff records were reviewed. The inspector arrived at the center at 11:15 am and departed at 4:30 pm.
Standard #: VENDSUB-000-006-B Description: Based on record review and interview with the director, the center failed to ensure that there was a written allergy care plan for each child with a diagnosed food allergy. Evidence: 1. There were five children with a diagnosed food allergy. None of the children had an allergy care plan with instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction. Plan of Correction: The director will research different model forms to see which has the information required to be documented. The director will have the parents take the forms to the physicians to have them completed. For future children enrolled with diagnosed allergies, the front desk operator will provide the parent with the appropriate form to have it completed by the physician prior to the child's first day of enrollment.
Standard #: VENDSUB-000-031-B-3 Description: Based on review of policies and procedures and interview with the director, the center failed to ensure that the lock down procedures included all required components. Evidence: 1. The lock down plan was missing the following components: methods to account for all children in the lock down location, methods of communication with parents, accommodations or special requirements for toddlers and children with special needs to ensure their safety during lock down, procedures to address reuniting children with parents or authorized person designated by parents to pick up the child. Plan of Correction: The director will add all of the required components to the plan and train staff.
