Discussed response on injury reports- "Review Safety Procedures" as comment to prevent recurrence on accident.
An unannounced afternoon inspection was conducted from 425 - 520. There were 58 children present with staff and they were located in various areas of the building. Homework, reading, art, and play were observed. The Inspector reviewed the evacuation drills, emergency preparedness plan, medication log, Operations manual, children's records, and attendance. The Inspector also observed transition to the gym and staff communicating about parent pick up. The CS is absent for the week but there is a MAT certified staff present and staff were available to help answer questions. Please call with questions 703-479-4679
Standard #: 22VAC40-185-40-J Description: Based on record review, the program has not updated the Injury Prevention plan annually. The last update has a documentation date of December 1, 2016 but there was nothing changed on the form. Plan of Correction: We will update based on accident reports.
Standard #: 22VAC40-185-340-F Description: Based on observation, the staff did not ensure children were in sight and sound supervision at all time. Evidence: 1. The Inspector arrived around 425 and rang the bell. The Inspector heard children in the door but sign said children were in the cafeteria until 430. The Inspector then walked around front of building and entered to find a staff member, sitting at a table with students in one room. The Inspector then noticed children in the second room and went into that room. There were 5 children and no staff present. One child was sitting in the rocking chair and reading from a lap top laptop, two were playing, and two were going through a box. 2. The staff member then came in and asked why they were in there and had them come into the other room. Plan of Correction: Talk with all the students and have staff remind children not to exit a room without staff supervision.
