The licensing inspector conducted a complaint inspection on February 16, 2017 regarding staff to children ratios. The inspector interviewed staff, made observations at the center, and reviewed documentation. The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid.
Standard #: 22VAC40-185-350-E-5 Complaint related: No Description: Based on record review and staff interviews, the licensee did not ensure the required staff to child ratio was maintained for school age children. One staff member for every 18 children present is required. Evidence: 1- Documents were reviewed on February 16, 2017, the licensing inspector reviewed head count forms for February 14, 2017 , there were 2 staff with 43 children from approximately 3:21pm to 3:55pm. 2- Documents were reviewed on February 16, 2017, the licensing inspector reviewed head count forms for February 15, 2017 there were 2 staff with 39 children from approximately 7:56 am to 8:30am . 3- Staff #1 and staff #2 were not able to confirm the dates of ratio concerns. Plan of Correction: Staff will ensure the ratio required by state licensing is being followed.
