Comments:
An unannounced monitoring inspection was conducted beginning at 8:10 a.m. and ending at 10:30 a.m. Children were observed having free play and at arrival. Three classrooms and supplies were observed. After free play, children completed math worksheets or continued free play at different centers that included imagination play and blocks. After free play and worksheets, children had circle time that included a story read by the teacher and reciting the alphabet and phonics practice. The playground was inspected. Five medications and authorizations were reviewed. Four children's records and two staff records, including one new staff, were reviewed. Emergency drills were reviewed. Standard #: 22VAC40-185-130-A
Description: Based on review of documentation, the center failed to obtain documentation of immunizations prior to the first day of attendance. Evidence: Child #1, start date 12/7/2015 and Child #2, start date 4/19/2016, did not have documentation of immunizations in the record.

Plan of Correction: The Immunization for this child was received before the inspection was finished.

Standard #: 22VAC40-185-140-A
Description: Based on review of documentation, the center failed to obtain documentation of a physical within thirty days of attendance. Evidence: Child #2, first day of attendance 4/19/2016, did not have a physical in the file.

Plan of Correction: This form was brought to us the next school day, and is up to date now.

Standard #: 22VAC40-185-160-A
Description: Based on review of documentation, the center failed to obtain a Tuberculosis (TB) test or screening within 21 days or 12 months of employment. Evidence: Staff #1, start date 1/4/2017, TB was dated 10/16/2015.

Plan of Correction: This employee had a TB test that was not yet expired, but did not fall into the category?s of acceptable by Licensing terms. This employee has since received a new TB test

Standard #: 22VAC40-185-40-G
Description: Based on review of documentation, the center failed to obtain proof of age and identity within seven days of attendance. Evidence: Child #3, enrolled 4/19/2016, did not have documentation of proof of age and identity.

Plan of Correction: We did originally have this form, but the parent took it to enroll their child in another program.

Standard #: 22VAC40-185-60-A
Description: Based on review of documentation, not all children's records were complete. Evidence: Child #3, enrolled 1/5/2016, did not have 1. an emergency contact other than the parent/guardian and 2. a full address for the second emergency contact

Plan of Correction: We have thoroughly reviewed all emergency contact forms to make sure all information is up to date and complete. As well as checking to make sure that the appropriate contact information is in the correct spaces.

Standard #: 22VAC40-185-270-A
Description: Based on observation, not all areas of the center were maintained in safe condition. Evidence: On the playground, there are 15 bolts on the wooden swing set that exceed two thread lengths, approximately six to eight thread lengths, that create a protrusion/impalement hazard.

Plan of Correction: The bolts have already been scheduled to be trimmed to the appropriate length, and the swings will remain closed until they are up to code.

Standard #: 22VAC40-185-510-C
Description: Based on a review of the medication, the center failed to follow it's procedures for medication. Evidence: Child #4's medication authorization for an epi pen jr was dated 9/30/2015 and was prescribed for "school year". An updated authorization from the parent and physician for the 2016-2017 was not in the file.

Plan of Correction: This form was brought to us the next school day, and is up to date now.

Standard #: 22VAC40-185-550-D
Description: Based on review of documentation, the center failed to document a minimum of two shelter-in-place drills per year. Evidence: There were no shelter-in-place drills documented for 2016.

Plan of Correction: We did to the shelter in place drills, but did not document them, and will make sure to document them in the future.

Standard #: 22VAC40-191-60-C-1
Description: Based on review of documentation and interview with the director, the center failed to obtain satisfactory background check within thirty days of employment. Evidence: Staff #1, start date 1/4/2017, did not have satisfactory criminal history results in the file.

Plan of Correction: We had background checks from the employees previous place of work, and were not aware that they did not count while we waited for our own forms to come back. The employee was not permitted to work with the children until we have all the forms back.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of documentation and interview with the director, the center failed to obtain satisfactory background check within thirty days of employment. Evidence: Staff #1, start date 1/4/2017, did not have satisfactory central registry results in the file.

Plan of Correction: We had background checks from the employees previous place of work, and were not aware that they did not count while we waited for our own forms to come back. The employee was not permitted to work with the children until we have all the forms back.
