An unannounced renewal inspection was conduced today 3/6/2017 and 3/13/2017. The inspector arrived at 8:00 A.M. and departed the program at 10:00 A.M. on 3/6/2017 and arrived at 8:30 A.M. and departed at 2:30 P.M. on the 13th. There were one hundred ten children present with nineteen direct care staff members. During the inspection all eleven classrooms were observed, lunch time was observed, infant care was observed, diapering was observed, hand washing was observed, nap time was observed emergency supplies were viewed, two medications along with authorizations were reviewed, policies and procedures were reviewed, and a sample of injury reports were reviewed.
Standard #: 22VAC40-185-160-A Description: Based on review of eight staff's records, the center failed to ensure that all records were complete with a TB screening with in 21 days of hire. Evidence: 1. The inspector found that staff member # 8 was hired on 1/11/2017 and obtained the TB screening on 3/7/2017. This was over the required 21 days allowed. Plan of Correction: The Center has implemented a new policy for hiring new staff.
Standard #: 22VAC40-185-280-H Description: Based on observation the center failed to ensure that all purses or pocket books were not accessible to the children in care that contained harmful agents. Evidence: 1. The inspector observed staff member # 8's purse sitting in the preschool area accessible to the children. The purse contained cigarettes and a lighter. Plan of Correction: The subject staff member locked her purse during the inspection. The subject staff member will not work at the Honey Tree Riverwalk location in the future, and all staff will be counseled and in serviced on this regulation.
Standard #: 22VAC40-185-310-B Description: Based on observation of the preschool classroom, the center failed to ensure that each classroom had 25 square feet per child. Evidence: 1. The inspector found 20 children in the preschool classroom. The classroom measured 22.3x18.11 for a total of 403.85 square feet. This space can have 16 children and two adults. Standard #: 22VAC40-185-350-E-1 Description: Based on observation of the infant room, the center failed to ensure that all required ratio were met. Evidence: 1. The inspector observed in the Little Lambs III room nine infants with two staff members from the time of arrival at 8:00 A.M. to the time of departure on the 3/6/3017 at 8:45 A.M. The required staff to infant ratio is one staff to four infants. Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 22VAC40-185-350-E-4 Description: Based on observation and interviews with staff of the preschool classroom, the center failed to ensure that all room were in the required staff to children ratio. Evidence: 1. The inspector observed the two preschool classroom and found they were out of ratio from 7:30 A.M. to 8:40 A.M. and also verified through interviews with staff. There were 17 preschool children ages 3-5 years of age in the first classroom with one staff person. The additional staff members arrived at 8:45 A.M. which placed this classroom into compliance with the ratio. The required ratio is one staff per 10 children. 2. The second preschool classroom had 20 children with one staff person. The second staff person arrived at 8:30 A.M. which placed this classroom into compliance. Plan of Correction: Honeytree respectfully submits as its plan of correction for this deficiency that it will revisit its "substitute teachers list" and procedures for responding with staff "call ins" That said, the events of that morning with two teachers being sick and two teacher running late were extremely atypical. Director's resume was attached for purposes of demonstrating that Honeytree employs a highly educated and experienced child care professional as the Director of this facility.
