An unannounced Subsidy Healthy and Safety Inspection was completed 3/13/2017. The inspector arrived at 8:30 A.M. and departed at 3:00 P.M. The Subsidy Agreement was signed on December 28, 2016 therefore all training requirements are to be completed within 90 days of signing. On the day of the inspection majority of staff currently have not completed the pre service training.

Description: Based on observation and interviews with staff, the center failed to ensure that all allergies , sensitivities and dietary restrictions were posted in a way that was immediately accessible to staff. Evidence: 1. The inspector found that one child was present with an known food allergy and the allergy was not posted.

Plan of Correction: The director will post all food allergies and will obtain parental authorizations.

Description: Based on review of one child's record that has a known food allergy, the center failed to ensure that a written allergy care plan for the child with a diagnosed food allergy,to include instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: 1. Child # 1 has an known food allergy and does not currently have the required written allergy plan from the doctor regarding the food and steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: The director will contact parent to get the written doctor's allergy plan.

Description: Based on review of the emergency plan, the center failed to ensure that the lock down drill was developed. Evidence: 1. The inspector found that the lock down procedure was not developed into writing. The director has an establihsed plan if a lock down drill needs to occur, but does not have it in writing.

Plan of Correction: The director will put the procedure in writing and ensure that all staff are aware.
