22VAC40-191 Background Checks (22VAC40-191)

An unannounced monitoring inspection was conducted today, March 21, 2017 to determine the center's compliance with licensing standards. The inspector arrived at 9:40 am and concluded the inspection at 12:15 pm. Upon the inspectors arrival, children were observed in a variety of activities. Children in the two primary classrooms were outside on the playground. Children played with balls, frisbees and on the climbing equipment. Upper and lower elementary were observed in their classrooms working on daily work cycles of math, science and Spanish. Some children were observed reading. Later the Upper and lower elementary children had time outside and the primary children returned to their classrooms where they listened to the story "Pinkalicious" and conducted hand washing and bathroom breaks to prepare for lunch. Children provide their own meals which were labeled with names. The primary children are working on their science fair projects. One classroom had the stems of multiple carnation flowers in different colored water to show the process of photosynthesis. All areas of the facility were inspected. The facility has age and stage appropriate materials and supplies. The facility had the following posted: License, last inspection, emergency numbers, evacuation maps/procedures, daily schedule and various parent information. The first aid kit and non-medical emergency supplies were inspected and found complete on both floors of the facility. The playground was inspected. The inspector interacted with staff and children in care. Two staff records and four children's records were reviewed during today's inspection (see violation notice). The following ratios were observed in each classroom: Lower elementary: (6-9 years): 28 children with 2 staff. Upper elementary: (9-12 years) 12 children with 1 staff. Primary I: (2.5-5 years) 17 children with 3 staff. Primary II: (3-5 years) 20 children with 2 staff. The last evacuation drill was conducted on 02/23/2017. A shelter-in-place drill for 2016 year was conducted on 03/07/2016. A second drill will need to be conducted by the end of the school year. The facility has current liability Insurance. The Emergency Preparedness plan is current. Health Inspection: 12/19/2016 Fire Inspection: 11/07/2016 Medications on site was reviewed. Two staff have current MAT certification. Staff have current CPR & First aide training along with Daily Health Observation training. The violations from the previous inspection on December 6, 2016 were checked for corrections. Description: Based on review of staff records, the facility did not obtain all required information for each staff record. Evidence: The record of staff #1, hired 08/31/2015 did not have documentation of two references obtained before employment as required.

Plan of Correction: Per the Assistant Director: We will document references within the time period required.

Standard #: 22VAC40-185-90--A
Description: Based on review of children's records, the facility did not obtain documentation of the written agreement between the parent and the center by the first day of the child's attendance. Evidence: Child #4, enrolled 10/03/2016, did not have documentation of the parental agreement signed by the parent that included a statement that (1) the center will notify the parent when the child becomes ill and (2) that the parent will notify the center within 24 hours after his child or any member of the household develops any reportable communicable disease.

Plan of Correction: Per the assistant director: I will have the parent complete and sign our infection control statements.

Standard #: 22VAC40-185-240-A
Description: Based on review of staff records, the facility did not ensure staff received orientation training by the end of their first day assuming job responsibilities. Evidence: Staff #1, hired 08/31/2015, did not have documentation of orientation training by the end of the first day assuming job responsibilities.

Plan of Correction: Per the assistant director: I will document orientation training for staff.
