Comments:
Thank you for your assistance during this unannounced renewal inspection. This on-site visit was conducted on March 22, 2017 from 10:50 a.m. through 2:10 p.m. On this date, there were fifty-three (53) children in the care of twelve (12) staff members in five classrooms. There are a total of sixty-five (65) children enrolled in this program. Five (5) children's records and fifteen (15) staff records were reviewed and updated. Areas of non-compliance may be found on the violation notice. Risk scores were discussed. Areas observed were; educational activities, infant feeding, hand washing/bathroom and diapering procedures, outside play, lunch, transition and nap. All interactions between staff and the children were appropriate and the children seemed comfortable with the daily routine. There are a wide variety of age/stage materials for the children in care. Children's artwork is displayed throughout the classroom. Standard #: 22VAC40-185-350-E-1
Description: One teacher was supervising five napping infant children. The ratio is one staff to four children ages birth to sixteen months. Evidence: Based on observation and conversation with the teacher it was confirmed one staff member was supervising five infant children. !. At 11:11 a.m. five infants were observed sleeping in the care of one staff member. 2. Two staff members were outside the napping area room supervising five infant children.

Plan of Correction: Ratio requirements were reviewed with staff members. Staff will make sure the ratio of one to four is being maintained at all times.

Standard #: 22VAC40-191-40-D-1-C
Description: Staff C did not have documentation of completing an updated sworn statement. Updates are to be completed every three years. Evidence: Base on record review and conversation with the assistant director, it was confirmed Staff A had failed to complete an updated sworn statement.

Plan of Correction: Staff A is absent today but will complete an updated sworn statement upon returning to work.
