Comments:
An unannounced subsidy inspection was conducted on 3/22/17 at 2:45 p. m. and concluded at 4:15 p. m. The inspector met with the director to review subsidy guidelines.

Violations:
Standard #: VENDSUB-000-006-B
Description: Based on review of the children records the center did not have all required information on file. Evidence: On 3/22/17, child #1 record was missing a written allergy plan for diagnosed food allergy.

Plan of Correction: Will get this from the parent.

Standard #: VENDSUB-000-031-A
Description: Based on discussion with the director, the center does not have all required written emergency plans. Evidence: On 3/22/17, there was no written plan/procedures for relocation or a lockdown.

Plan of Correction: Will develop written emergency plans for relocation and lockdown.
