Comments:
The licensing inspector conducted an unannounced inspection on Thursday, March 23, 2017 from approximately 10:30am to 12:40pm. There were 15 children present being supervised by two staff members. During the inspection, the children were observed eating lunch, playing with balls in the gym, sorting toys by colors, taking bathroom breaks and washing their hands. Lunch consisted of hamburgers, oranges, sweet potato fries and milk. Later children transitioned into nap time. Areas of the facility including the classroom, hallways, bathroom, gym and outdoor play area were inspected. The facilities were equipped with toys and supplies and items were available to the children.

The following information was reviewed: emergency supplies, emergency preparedness plan, menu, injury prevention plan, daily attendance, parent agreements and staff training information. Required items were posted in the classroom.

During the inspection, three children's records and two staff records were reviewed. The required information was on site for staff members. The licensing inspector will schedule a time at a later date to review additional staff information. Standard #: 22VAC40-185-60-A
Description: Based on review of three children's records, one record did not contain the required information.

Evidence:
During the inspection, the licensing inspector observed the two emergency contacts listed in the record of Child #1 documented a post office box address. Each child's record is required to contain name, physical address, and phone number of two designated people to call in an emergency if a parent cannot be reached.

Plan of Correction: Another form will be sent home today with the child to obtain the corrected information.

Standard #: 22VAC40-185-550-E
Description: Based on interview and documentation review, the center did not maintain record of the dates of the practice drills for at least one year. Evidence: During the inspection, the licensing inspector asked to review emergency drills. The staff had documentation of a completed shelter-in-place drill dated 03/21/2017. Staff stated the documentation of evacuation or fire drills was sent to the main office and has not been returned.

Plan of Correction: We will start keeping a copy of the fire drills here at the center.
