Comments:
An unannounced monitoring inspection was completed on March 16, 2017. The purpose of this visit was to measure the facility?s compliance with the licensing standards. The following ratios were observed: Infants- 1:3; Toddlers-2:7; Young Preschool- 2:7; Preschool-2:11.
The infants were napping. The toddlers were watching Super Why. The young preschool children were playing outside. The older preschool children were also observed working on their fine motor skills by practicing paper cutting with scissors. The children ate breaded fish sticks, applesauce, corn and milk for lunch. The facility administers medication. There was currently no medication on site. The first aid kit and emergency supplies were inspected. The last evacuation drill was conducted 3/13/17. The most recent shelter in place drill was conducted 9/30/16. Four children?s records and five staff records were reviewed.

Standard #: 22VAC40-185-160-A
Description: Based on a review of new staff records, the facility did not ensure that new staff submitted results of tuberculosis(TB) screening within 21 days of employment. EVIDENCE: The records of staff persons # 2 (hire date-1/17/17), 4 (hire date- 2/10/17) and 5 (2/6/17), did not contain TB test results.

Plan of Correction: The staff files have been updated and the employees that are over 21 days will not work until TB test is completed.

Standard #: 22VAC40-185-270-A
Description: Based on observations at the facility, the facility did not ensure that all areas and equipment inside and outside of the facility were maintained in a clean, safe and operable condition. EVIDENCE: 1) The inspector observed dirty bouncers and a high chair that were all soiled with crusty food and food particles. 2) The inspector observed a broken red plastic basket used to store toys, that was broken exposing jagged and sharp edges. 3) The inspector observed two broken electrical faceplates in the preschool/ school age building. One at the rear entrance on the preschool side of the room and the other was located near the front window on the school age side of the room. The damage makes the insides of the electrical hardware accessible to children. 4) The inspector observed a nail protruding from the wooden fence in the outdoor play area. There was also an area in the play space where the asphalt meets the dirt that is uneven and presents a trip hazard for children and staff.

Plan of Correction: The bouncers were washed and sanitized. Broken toys were disposed. The electrical faceplates were replaced. The nail was removed and dirt was added to the play area.

Description: Based on observations in the infant area, the facility did not ensure that cribs were placed at least 12 inches apart while in use. EVIDENCE: The inspector observed infants sleeping in cribs that were placed less than 12 inches from each other.

Plan of Correction: The staff will leave cribs 12 inches apart everyday.

Standard #: 22VAC40-185-500-B
Description: Based on observations of diapering procedures, the facility did not ensure that proper cleaning procedures were followed and that all required items are located at the diaper changing sink. EVIDENCE: 1)The inspector observed diaper changing. The staff person failed to wash the changing pad with a soap and water solution first before sanitizing. 2) There was no soap at the sink in the diapering area.

Plan of Correction: I will review with new staff diaper changing procedures.

Standard #: 22VAC40-185-540-E
Description: Based on observations at the facility, the facility did not ensure that the preschool/school age building had required nonmedical emergency supplies. EVIDENCE: The preschool/ school age building did not have a flashlight or battery operated radio.

Plan of Correction: A flashlight and radio was bought for school age area.

Description: Based on a review of documentation, the facility did not ensure that two shelter in place drills were conducted in 2016. EVIDENCE: The facility conducted one shelter in place drill in 2016 on September 30th.

Plan of Correction: I will make sure two drills are done each year.
