An unannounced monitoring inspection was completed on March 23, 2017. The purpose of this visit was to measure the facility?s compliance with the licensing standards. The following ratios were observed: Pre-K 3- 1:9; Pre-K-4- 2:11; Pre-K-5-2:15. The children in Pre-K 3 were observed playing on the playground. The children in Pre-K 4 were participating in letter sound review with emphasis on the letter ?o?. The children were read the book, Owl Lake. The Pre-K 5 children were observed working in small groups. Some of the activities included letter writing, building with blocks, addition with manipulatives and an activity based on the book 10 Little Rubber Ducks. The children bring their own snack and lunch. Staff was observed interacting with the children and providing assistance when needed. All areas of the center including classrooms, hallways, bathrooms and playgrounds were inspected. The facility was equipped with toys and supplies and items were available to the children. Medication is administered; however there is currently no medication on site. During the inspection, three children's records and two staff records were reviewed. The first aid kit and emergency supplies were inspected. Violations cited at the last inspection were verified as corrected. Approximate time on site: 10:30am to 1:15pm
Standard #: 22VAC40-185-330-B Description: Based on an inspection of the playground area, the facility did not ensure that the resilient surfacing was in compliance with minimum safety standards. EVIDENCE: The pea gravel located at the base and sides of the yellow slide measured at approximately 4 inches at 4 different points. The pea gravel should be at least 6 inches in depth in the fall zone surrounding climbing equipment and equipment with moving parts. Plan of Correction: We will take care of the surfacing within 10 days.
Standard #: 22VAC40-185-540-E Description: Based on an inspection of non medical emergency supplies, the facility did not ensure that all equipment was in working order. EVIDENCE: The battery operated radio worked only while plugged into a power source. The radio must also be able to operate with the use of batteries alone. Plan of Correction: Corrected on site.
Standard #: 22VAC40-185-550-D Description: Based on a review of evacuation drills, the facility did not complete evacuation drills on a monthly basis. EVIDENCE: The last recorded evacuation drill was 1/16/17. There was no recorded drill in February 2017. Plan of Correction: We will perform monthly evacuation drills and will document immediately following the drills.
