22VAC40-191 Background Checks (22VAC40-191)
An unannounced monitoring inspection was conducted from 9:30am-11:15am. Upon arrival there were 135 children in care with 24 staff present. A tour of the facility was conducted in addition to records reviewed and interviews with staff and groups of children. A sample of 2 staff and 5 children's records were reviewed in addition to 2 medications. Transportation was reviewed with management. Children were observed playing inc centers, eating morning snack and listening to music.
Standard #: 22VAC40-185-270-A Description: Based on observation the center failed to ensure areas and equipment of the center, inside and outside, were maintained in a safe condition. Evidence: 1. On the playground there were 3 red tricycles that had rust on the metal and peeling paint and rust on the green box next to the fuse box. 2. In the Kindergarten hallway there was a hole on the wall in reach of children. A child's finger can fit inside. Plan of Correction: The director will contact maintenance to remove the rust on the trikes and the green metal box. The metal box will be stripped and repainted. The hole will be covered with tape until it is patched.
