Comments:
The Licensing Inspector conducted an unannounced annual subsidy health and safety inspection today, March 28, 2017 from approximately 10:00 am to 3:00 pm. The facility director/ administrator participated in the inspection. During the inspection, children and staff were observed participating in a variety of activities. Activities among the preschool children included center time with tabletop toys, puzzles, blocks, art (cutting out pictures from magazine and gluing them on paper) sensory table, and books). Pre-kindergarten and Jr. kindergarten classes had a visitor who presented a program on recycling and later the classrooms had time on the playground. Lower and upper nursery were observed exploring with prisms and sensory tubes. Children and staff discussed the colors while looking through tented glass. Toddler played with bubbles and dolls while staff conducted diapering. Hand washing was observed. Later lunch time and nap time were observed. Staff interacted in a positive manner with children. All areas of the facility to include classrooms, kitchen, halls, bathrooms, outdoor play areas and the facilities van/ buses were inspected. The facility was equipped with age and stage appropriate materials and supplies. Additional subsidy requirements that were not addressed during the renewal inspection were reviewed. During the inspection, four staff records and eight children's records were reviewed. Ratios Observed: Toddlers: 7 children with 2 staff. Half-Day Preschool: 10 children with 2 staff. Jr. Pre-kindergarten: 9 children with 2 staff. Pre-kindergarten: 13 children with 2 staff. Pre-school: 12 children with 4 staff. Upper Nursery: 4 children with 1 staff. Lower Nursery: 6 children with 2 staff. Today's lunch consisted of ham and cheese sandwich, green beans, pears and milk. Description: Based on record review and interview, the facility did not ensure a child with a diagnosed food allergy had a written allergy care plan that included instructions from a physician regarding the food to which the child is allergic to and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: 1. During the inspection, the licensing inspector reviewed the record for child #1 (enrolled 09/08/2009). The record contained statements by the parent that child #1 had food allergies however the record did not contain a written allergy care plan by a physician that included all requirements. 2. During the inspection, the licensing inspector reviewed the record for child #3 (enrolled 09/08/2015). The record contained statements by the parent that child #3 had food allergies however the record did not contain a written allergy care plan by a physician that included all requirements.

Plan of Correction: Per the Director: I have an food allergy action plan form that i will give to parents today to have completed by their physician. I will request that the plan be returned to me as soon as possible.
