VENDSUB-000 STAFF QUALIFICATIONS AND TRAINING
An unannounced subsidy health and safety inspection was completed on 3/30/17 as a part of the regular licensing monitoring inspection.
Standard #: VENDSUB-000-006-B Description: Based on record review, the center failed to ensure that there was an allergy action plan for one of one child with a diagnosed food allergy. Evidence: 1. There was one child with a diagnosed peanut, tree nut, and egg allergy. The child did not have an allergy action plan. The child had an Epipen and a completed Medication Administration form by the physician. Plan of Correction: The parent provided an allergy action plan completed by the physician during the inspection. The director will ensure that an action plan is provided for any future documented allergies.
Standard #: VENDSUB-000-012-B Description: Based on record review, the center failed to ensure that all staff completed the Virginia Preservice Training for Child Care Staff within the first 90 days of employment or subsidy vendor approval. Evidence: 1. The subsidy vendor approval was dated 11/28/16. There were two staff who had been employed more than 90 days (June 2002 and October 2016) who had not completed the 10-hour preservice training. Neither staff were direct care staff (bus driver and maintenance person). Plan of Correction: The director will have the staff complete the training. All future staff will complete the training within 90 days of hire.
Standard #: VENDSUB-000-012-E Description: Based on record review, the center failed to ensure that all staff had completed first aid and CPR training within 90 days of the subsidy vendor agreement (for current staff) or 30 days after the date of employment (for new staff). Evidence: 1.The subsidy vendor approval was dated 11/28/16. There were 17 staff who had been employed more than 30 days who did not have current documentation of certification in first aid and CPR. The director had documentation of eight staff who had attended a CPR training on 3/27/17 but had not completed first aid training. Plan of Correction: The director will provide first aid and CPR training to the staff over the next two weekends. All new staff will be trained within 30 days of hire. A training will be provided monthly for staff.
