Discussion was held on the topics of current enrollment and current staffing.
The following inspection took place between the hours of 11 am through 3 pm. Today, I met with the Staff of the Chesterbrook Academy #801 located in Fairfax, Virginia. At the start of this unannounced monitoring inspection there were a total of 126 children with 21 staff; good ratios in all 7 rooms. A total of 5 children?s files and 3 new staff files were reviewed. The children were engaged in the following: center activities within each room, story time, games, art, large motor skills on the playground, diaper changing, hand washing, bathroom break, lunch, nap time,and other organized activities. Standard #: 22VAC40-185-130-B Description: Based on review, 1 of 5 children's files reviewed indicated that the center did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence - 1. On the date of inspection (03/29/2017), the most current Immunization documentation on file for Child #2 was dated - 05/17/2016. 2. Based on the information on file, on the date of inspection, Child #2 was under the age of two years. Plan of Correction: An updated immunization page shall be obtained for Child #2 and placed on file.
Standard #: 22VAC40-185-280-B Description: Based on observation and interview, hazardous substances were not kept in a locked place. Evidence - 1. On the date of inspection (03/29/2017), items marked "keep out of reach of children" and "contact poison control if ingested" were accessible to the children in care. 2. The Center Director stated (during an interview conducted on the date of inspection) that the containers of hand sanitizer observed were for the staff's use only. Plan of Correction: Corrected on the date of inspection.
Standard #: 22VAC40-191-60-C-1 Description: Based on review, 1 of 3 new staff files reviewed indicated that child day center does not have an original criminal history record report (CRC) within 30 days of employment. Evidence - 1. On the date of inspection (03/29/2017), Staff #1 did not have an original CRC within 30 days of employment. 2. The start date for Staff #1 was listed as - 01/23/2017. Based on information contain within the file for Staff #1, the request for the CRC was not sent until 02/09/17. This date is not within seven days of their start date. 3. This request was returned on 03/02/17, and was resent - 03/06/17. 4. A call placed into the Central Criminal Records Exchange on 03/29/2017 indicated that this request has yet to be log in as received in their office. Plan of Correction: Current employment of Staff #1 shall be denied until the CRC has been obtained.
Standard #: 22VAC40-191-60-C-2 Description: Based on review, 1 of 3 new staff files reviewed indicated that the Child Day Center does not have a central registry finding (CPS)within 30 days of employment. Evidence - 1. On the date of inspection (03/29/2017), based on the information on file, the start date for Staff #3 was listed as 02/16/2017. 2. A copy of the CPS request was sent on 02/24/2017. This date was not within seven days of their start date. 3. However, a call to the Central Registry indicated that the request was received on 03/13/2017 and will take approximately 2 weeks until it is processed. Plan of Correction: Current employment of Staff #3 shall be denied until the CPS has been obtained.
