Comments:
Today's visit was a renewal inspection. This inspection included the subsidy health and safety inspection. The inspection began at 9:30am and concluded at 2:20pm. Upon arrival at the center there were twenty-five children in care with five staff members. During the inspection activities in the multipurpose room were observed, diapering was observed, all three classrooms were viewed, a sample of three out of twelve staff records were reviewed, a sample of three out of seventy-five children's records were reviewed, the outdoor area was viewed, nap time was observed, some policies and procedures were reviewed to include the new lock down procedure, and emergency supplies were viewed for all three levels of the building. Your renewal application has been received in the licensing office. A recommendation regarding your license will be made upon completion of today's inspection. Description: Based on the inspector's review of a sample of three out of seventy-five children's records, the center failed to ensure two children with diagnosed food allergies have a written allergy care plan. Evidence: The records for children #2 and #3 have documented food allergies. Both records do not include a written allergy care plan to include instructions from the physician regarding the food allergy and steps to be taken in the event of a suspected or confirmed allergic reaction.

Plan of Correction: The Program Director has requested from both of the parent to provide a written allergy plan from the child's physician.
