Discussed the appropriate type of diaper trash can to be used. Discussed ways to properly record the attendance in each classroom when the groups separate.

An unannounced monitoring inspection was conducted today. There were 50 children in care, with a total of 10 staff members assigned to their direct supervision. The children were observed playing board games, reading books, practicing their writing skills and math skills, and attending language classes. Standard #: 22VAC40-185-80-A
Description: Based on a review of administrative documents, it was determined that one classroom did not complete their written attendance. Evidence: The written attendance in Room 3 was not completed.

Plan of Correction: The attendance was updated during the inspection.

Standard #: 22VAC40-185-240-A
Description: Based on a review of staff training documentation, it was determined that two staff have not completed all required components for orientation training. Evidence: 1. For staff A, there is no documentation showing that she completed "Recognizing Child Abuse and Neglect" training. 2. For staff C, there is no documentation showing that she completed "Recognizing Child Abuse and Neglect" training, and First Day Orientation training.

Plan of Correction: All trainings were completed. The documentation will be placed in each file.

Standard #: 22VAC40-185-270-A
Description: Based on observations, it was determined that one classroom exit door was blocked. Evidence: In Room 3, one of the three classroom exit doors was blocked by a row of chairs.

Plan of Correction: The chairs were immediately moved during the inspection.

Standard #: 22VAC40-185-350-A
Description: Based on observations, it was determined that one staff was not directly supervising the children in her classroom as required. Evidence: When the Inspector entered Room 1, she observed one staff person concentrating on her cell phone rather providing direct supervision to the children in her immediate care. This staff person was distracted by her cell phone for approximately 30 seconds.

Plan of Correction: The staff person put the phone away when asked. In addition, all staff cell phones were immediately taken and put in the office in order for all staff to follow the school's policies of no cell phones permitted in the classroom.

Standard #: 22VAC40-185-540-B
Description: Based on observations, it was determined that one first aid kit was accessible to the children. Evidence: One small first aid kit was stored in a hallway cubby shelf.

Plan of Correction: The first aid kit was moved during the inspection.
