An Unannounced monitoring inspection was conducted beginning at 6:15 and ending at 7:00. As of 6:555, there were 19 children present with 5 staff. Children were participating in table activities as they arrived.. Reviewed staff files - see Violation Notice. Standard #: 22VAC40-185-240-A Description: Based on a review of 7 staff files, it was determined that one file did not have documentation of Orientation. Evidence: Staff #1, who began work at this site three weeks ago but who was working at another site in January, did not have documentation of her Orientation that should have been done as of her first day. Plan of Correction: Staff will get the documentation from the main office or if it is not available, will redo the Orientation Training.
