Standard #: 22VAC40-185-160-C Description: Based on review of a sample of staff records all follow up tuberculosis ( TB ) screenings were not maintained. Evidence: During inspection staff #2 was missing documentation of an up to date TB screening. Plan of Correction: That staff had gone to the doctor but has not turned in the form. I will talk to her this afternoon.
Standard #: 22VAC40-185-60-A Description: Based on review of a sample of children's records off required information was not maintained. Evidence: During inspection child #1 was missing : parents work phone number, proof of identification and immunization records. Child #2 was missing: 1 of 2 emergency contacts and parents work phone number. Child #3 was missing 2 of 2 emergency contacts. Child #4 was missing: Addresses for 2 of 2 emergency contacts,work phone number for 1 of 2 parents, proof of identification and immunization record. Plan of Correction: Today is Child #'s last day. I will work on this today
Standard #: 22VAC40-185-270-A Description: Based on inspection of Child Day Center all areas were not maintained in a clean condition. During inspection toilets in the 3's class room were used and not flushed. Plan of Correction: During inspection director corrected.
Standard #: 22VAC40-185-280-B Description: Based on inspection of Child Day Center cleaning materials were not kept in a locked place. Evidence: During inspection cleaning agents were stored in unlocked cabinets in the 4 year old room and Infant 1 rooms. Plan of Correction: Director corrected as pointed out by inspector on inspection tour.
