An unannounced monitoring inspection was done today April 3, 2017 beginning at 8:50 AM and ending at 2:30 PM. The infants were sleeping and being fed. The inspector reviewed a sample of medications, children and staff files. Lunch and nap were observed and a physical plant inspection was completed. Discussion with director and staff about: using a high chair according to the manufacture recommendations for age and size, labeling personal items with owners names, numbering cots and cloth diapers process.
Standard #: 22VAC40-185-160-A Description: Based on review of three staff's records, the center failed to ensure that all TB screening were completed within the 21 days of hire. Evidence: 1. The inspector observed that staff # 1 was hired on 11/21/2016 and the TB screening was completed on 1/20/2017. Plan of Correction: In the future all time lines will be followed and monitored.
Standard #: 22VAC40-185-270-A Description: Based on observation the of the classrooms, the center failed to ensure that all areas inside and outside the center shall be maintained in a clean, safe and operable condition. Evidence: 1. The inspector found on the toddlers and two years door that the lock button is missing and there is a medal protrusion sticking out from the handle. Plan of Correction: The director staff will either purchase new knobs or will have new locks placed on the the door. All areas will be monitored.
Standard #: 22VAC40-185-340-D Description: Based on observation and record review, the center failed to ensure that all groupings of children had a qualified program lead. Evidence: 1. The inspector reviewed staff # 2 record and found that the staff person did not meet the requirements for program lead. On the day of the inspection staff # 2 was acting as program lead in the 3 year old classroom. Plan of Correction: The director will make sure that all substitutes will have program leader qualifications when they are placed in a program leader role.
Standard #: 22VAC40-185-340-F Description: Based on observation, the center failed to ensure that all children under the age of 10 years shall be within actual sight and sound supervision at all times. Evidence: 1. The inspector observed the 3/4 year old teacher walking out of the bathroom that was across the hall from the classroom. There were eight children in the classroom that were left out of sight supervision. The amount of time could not be determined. The teacher stated that she was turning off the water from the sink. Plan of Correction: The director will speak with the teacher and will monitor.
Standard #: 22VAC40-185-520-C Description: Based on observation of the two year old room, the center failed to ensure that all requirements for the diaper ointments and cream were followed. Evidence: 1. The inspector observed one diaper ointment container that expired 3/2014 and did not have a name on the container. Plan of Correction: The container was discarded during the inspection. All diaper creams will be monitored.
