Comments:
Licensing Inspector arrived at 9:55am to complete an initial inspection. No children were in care, thus those standards regarding the care and supervision of children were not reviewed. All spaces to be used by the children were measured and inspected. The application requests a capacity of 150 which is not supported by the measurements taken at the inspection today. Based on the measured usable space, the facility can have a capacity of 141 children. The playground space for the facility was measured and inspected. One staff record was reviewed to determine the qualifications for the Program Director. No records were reviewed for children at the inspection today. Any areas of standards related to direct service that were not determined during the initial inspection will be determined at the first unannounced inspection following the issuance of a license. All future inspections will be unannounced. The first unannounced inspection will be conducted within 60 days from the date the license is issued. All areas of non-compliance are identified on the violation notice attached to this summary and were discussed at the exit interview.

Standard #: 22VAC40-185-250-C
Description: Based on observation, it was determined that the facility did not ensure that a notice regarding the presence and location of asbestos containing materials and advising that the asbestos inspection report and management plan are available for review shall be posted. Evidence: 1. During a tour of the entire facility, the Licensing Inspector was unable to locate a notice posted regarding the presence of asbestos in the facility and it's location. 2. Staff #1 (Program Director) confirmed that there was no asbestos statement posted in the facility.

Plan of Correction: The facility responded: On 4/5/17, a notice was posted in the lobby stating that asbestos was found in the main closet located in the back of the center, and that the asbestos management plan can be found in the director's office.

Standard #: 22VAC40-185-540-A
Description: Based on a review of the first aid kits for the facility's vans and interviews, it was determined that the facility did not ensure that all first aid kits include all of the required items. Evidence: 1. The first aid kit for the facility only included one triangle bandage. 2. Staff #1 (Program Director) confirmed that the first aid kit for the facility did not include all of the required items.

Plan of Correction: The facility responded: On 4/5/17, management added an additional triangle bandage to the First Aid Kit to create a complete kit. Management will routinely check the First Aid Kit to ensure it is complete.
