Description: Based on observation and interview the center failed to ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children. Evidence: 1 - There are three spray cans of disinfectant on a top shelf in the back classroom. This is an unlocked location. The cans each have labels that read caution, hazardous to humans. 2 - There are pump bottles of liquid soap sitting on the sinks in two of three bathrooms used by children in care. The labels on the bottles read warning, keep out of reach of children and avoid getting in eyes. 3 - Staff 1 confirmed that the disinfectant and liquid soap were stored in unlocked places.

Plan of Correction: Director and staff will discuss the violation and make sure all disinfected spray bottles will be locked. Director will provide soap bottles without any warning, therefore will be save for children.
