Technical Assistance:
During inspection inspector discussed playground safety procedures making and sure teachers know to actively monitor children. Also, center director was informed that children may not be routinely moved to other classrooms to maintain ratios .

Comments:
An investigation and inspection was conducted today from approximately 3:40pm-6:30pm in response to a self-reported incident received by our office regarding the care and protection of children on the playground. Based upon interviews and record reviews the preponderance of evidence does not support a violation pertaining to the incident at this time. There were 36 children (infants-school age) directly supervised by 7 staff. The physical plant, 4 staff records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books and participating in craft activities. Diapering and hand-washing procedures were also observed. There was an adequate supply of materials for the children. Areas of non-compliance are identified in the Violation Notice. Description: Based on review of a sample of children's records all required information was not maintained. Evidence: During inspection Child #4 was missing parents' work phone number.

Plan of Correction: communicate with parent to complete necessary information in file

Standard #: 22VAC40-185-270-A
Description: Based on inspection of Child Day Center all equipment was not maintained in a safe and operable condition. Evidence: During inspection the trash can in the 3's classroom had a broken foot pedal.

Plan of Correction: Removed trash can and replace with new equipment.
