An unannounced subsidy health and safety inspection was completed on April 18, 2017. The purpose of this visit was to measure the facility?s compliance with the subsidy vendor requirements. The following ratios were observed: Infants-1:4; Toddlers-1:4; Two year olds-1:5; Three year olds-1:6; Four year olds- 1:5
The children throughout the facility were engaged in activity. The infants were eating, sleeping, and playing on the floor. Children in toddler through four year old classes were observed coloring, listening to stories, playing with toys and writing sight words. Staff was observed interacting with the children and providing assistance when needed. All areas of the center including classrooms, hallways, bathrooms and playgrounds were inspected. The facility was equipped with manipulatives and supplies and items were available to the children. The following information was reviewed: emergency preparedness plan, fire and shelter-in-place drills, injury prevention plan, liability insurance policy, parent agreements, and staff training information. Medication is administered; however, no medication is currently on site. Staff is certified in cpr/first aid, daily health screening and MAT. The first aid kit and emergency supplies were inspected and found to be complete.

During the inspection, three children's records and thee staff records were reviewed. Staff employed as of this date has completed the pre-service training.

Standard #: VENDSUB-000-031-B-1 Description: Based on a review of the emergency plan, the facility did not ensure that the emergency plan included all required components. EVIDENCE: The evacuation plan did not include accommodations for infants, toddlers and children with special needs or procedures to address reunification with parents or other authorized person. Plan of Correction: We will make sure to update the emergency plan to include required evacuation components.

EVIDENCE: The shelter-in-place procedures did not include accommodations for infants, toddlers and children with special needs or procedures to address reunification with parents or other authorized person. Plan of Correction: We will update the emergency plan to include all shelter-in-place components.

EVIDENCE: The facility's emergency preparedness plan did not contain lockdown procedures. Plan of Correction: We will add the required lockdown procedures to the plan.
