22VAC40-191 Background Checks (22VAC40-191)

Technical Assistance:

An unannounced renewal inspection was made today, 4/19/17 at 10:00 a. m. and concluded at 2:00 p. m. The inspector met with the staff and Child Day Center standards were reviewed. The following staff-to-child ratios and activities were observed: Infants: There were eight infants present with two staff. Two infants were napping and six infants were awake and playing on the floor, the staff were observed positively interacting with the infants. Older Infants: There were four infants with one staff. The infants were observed watching an educational video and doing finger plays, "Twinkle, Twinkle Little Star". Toddlers: There were nine children with two staff. The children were playing on the rug with the staff, they were playing "Roll the Ball". Older Toddlers: There were four children with one staff. The children and staff were playing a game with the balls. Two Years: There were eight children with one staff. The staff was having circle time and reading the children a story. Three Years: There were ten children with one staff. The staff was having circle time and reading the children a story. The other Three and Four Year classes were on field trip.. Ten staff records were reviewed. Medications were checked for three children. Several staff are trained in MAT (medication administration trained). Four children records were reviewed. Conversations were held with several staff and children. Classroom observations were conducted. The last monthly evacuation drill and shelter-in-place drill was conducted last month.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not have a toy outside maintained in a safe condition. Evidence: On 4/19/17, there was a plastic flower missing at the trunk of the Little Tikes tree house. The stem of the flower was protruding and could be a hazard.

Plan of Correction: The stem will be removed until the flower can be replaced. This will be corrected before children are allowed on playground.

Standard #: 22VAC40-185-440-N
Description: Based on observation the center had mobiles on cribs in the infant room for children over five months. Evidence: On 4/19/17, there were two cribs with mobiles and the infants napping in the cribs were older than five months of age.

Plan of Correction: The mobiles were removed from the cribs immediately while inspector was on site.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on review of records, the center did not have required background checks updated for application to be complete. Evidence: On 4/19/17, the criminal history record and the search of the central registry clearance was not updated on one person listed on the renewal application

Plan of Correction: The background checks were submitted on 3/31/17, waiting for them to arrive.
